Replacement Card Request Form
Please use this form to request a replacement Southern Co-op membership card and confirm your current postal address. (The replacement card features our white and orange honeycomb design with the Southern Co-op logo on the bottom left corner.) If you request a card, it will be sent to your confirmed address within 12–14 days.
